Stoneridge Hospice provides specialized care: Nursing services, Physician Care, Counseling, Therapy, Pain Management, DME and Supplies, Routine Hospice Care, Bereavement Services to patients diagnosed with 6 months or less to live throughout Surprise.  You can have your physician make a referral to us or you can call and speak with our medical team and/or Medical Director directly.
